Rates
The cost for treatment is dependent on services provided. Your initial visit is $125 and once we discuss your intake forms (download forms here), we get right into treatment. I accept credit cards, local checks, PayPal, Venmo, Zell, and cash.
Insurance
I accept the following insurance:
- Aetna/Meritain
- Anthem Blue Cross Blue Shield (BCBS)
- CHP+ (CO Access and Friday Health Plans of CO)
- Cigna
- Kaiser Permanente
- TriCare and TriWest
- United Health Care/UMR
- Victims Compensation (Fremont, El Paso, Pueblo counties)
- Employee Assistance Programs (EAP)
Cancellation and No-Show Policy
While I realize that issues come up that cannot be helped, please understand that my policy is about mutual respect. If you need to cancel or move an appointment, please do so within 24 hours of the appointment. No-shows and late cancelation (less than 24 hours) are subject to a $60 fee. If either of these policies are not honored, or late cancel or no-show charges not paid, services may be revoked.
